This form adds additional terms to your agreement with UAL to support the delivery of Insights workshops.
It also allows you to notify us of any special requirements you have, to confirm the details of your parent or guardian, and if you both choose, to enter into a separate agreement for UAL to use photographs or video of you in promotional material.

Privacy Notice
This notice sets out how UAL’s Outreach programme, UAL Insights, uses your personal information. Your information is collected when you register for one our programmes using our online forms.
University of the Arts London (UAL) will process the information you provide on this form and personal data collected or created as part of our relationship with you.
We will also process your health or other sensitive data under GDPR 9(2)(a) ‘explicit consent of the data subject’ (you, the student), where you wish to tell us of special requirements.

The use of photographs and video for publicity
We would like to use photographs or video of students to promote the Insights programme, either in our printed publications, website or social media accounts, or with external partners such as local press, or other cultural organisations.
If we use images of individual students or their work, we will not use the full name of that student in the accompanying text or photo caption. Feedback from tutors relating to the work may be included with the image.
We will only use photographs or video of students for these purposes if agreed by both the student and the parent / guardian.

Additional obligations of the student
- While taking part in Insights workshops and events I will take a professional attitude to my attendance, punctuality and presentation.
- I will be responsible for the language I use and only post comments that I would feel comfortable knowing anyone could read on Insights digital platforms.
- I will not engage in or send anyone material that could be considered threatening, bullying, harmful, offensive or illegal.
