Processing this information is necessary for the University to fulfil its obligations to you under the Equality Act (2010). We will store this information confidentially within the Disability Service database. This is separate from your student record.
We will not share any information that you provide with anyone outside the Disability Service without asking you for explicit permission first, unless the situation falls within the exceptions shown in the Confidentiality statement section (next page).
We will retain records of your interactions with the Disability Service for 6 years after you leave the University.

All forms are reviewed by a person who will be replying or putting in place support for you.
The course and personal information you provide may be automatically updated as our casework system checks the student record for any changes related to your course or personal details; for example, when you change your address or when you progress from one academic year to another.
You chose not to allow us to store your information. We require this permission so that we can support you.

We usually ask students to give their explicit permission to share information before we share it with others. We share information on a ‘need to know’ basis. This means we share the minimum information needed for another team, staff member or organisation to support the student.
Student Services may occasionally need to share information without a student’s permission. This might happen in an emergency or if the student or others are at risk of serious, lasting harm. The Dean of Students, who oversees Student Services, decides when to share information without a student’s permission.
The Disability Service may use the information that you provide to send you information that may be useful to your circumstances, later in your studies.

If you are a UK-based student, you will not need an English Language test.
UAL has a large international community. The Disability Service can provide support to overseas students. The Student Advice Service can provide more specialist advice about immigration and visas.
You may need to take a Secure English Language Test (SELT) to meet UK Visas and Immigration (UKVI) requirements before you can start your course. If you need to do this, it will be made clear in your offer. Visit UAL's webpages for further information about English language tests.
Please note: The Disability service cannot alter the language requirements for your course.
If you do need more support with immigration and visas, then please contact the Student Advice Service. They can provide more specialist advice about Immigration and visas.
If you need adjustments for the test, for example, additional time, you should also contact your test centre as early as possible. Your test centre may ask for evidence of your impairment or Specific Learning Difference. It can take some time to put adjustments in place.
The Disability Service is part of a larger group of Student Services.
By completing this form, you've already given us consent to store your information and share information with other Student Services departments.
We need your permission to share information about you with other groups outside of Student Services. This is called "Consenting to Share".
We ask for your consent so we can arrange the support you need. We only share information with the people who need it to provide that support. This is usually about practical things, not your diagnosis.
We won't pass any information on until you have spoken to a Disability Adviser.
