  • About this form

    To make a company booking onto a short course using a purchase order payment process, please complete this form.

    Please note: If you would like to make payment using a credit / debit card, please return to the course webpage and proceed using the online payment process instead. 

    This form allows you to book up to 2 courses each for up to 10 people. If you would like to book for more courses or candidates, please contact us on shortcoursecompanybookings@arts.ac.uk.

    What you'll need

    • Purchase order number or Company Headed Paper. Please ensure we are set up as a supplier on your accounting system, so that we can invoice you.
    • Scan of purchase order, or a sponsor letter from your company on headed paper.
    • Student nationality and date of birth.

    Privacy Statement

    What we need

    UAL Short Courses Ltd is the ‘Data Controller’ of the personal data that you provide to us. We are required to collect basic personal data about you including name, address, telephone, email, nationality and date of birth. We will also create personal data in relation to your participation in the course, such as tuition notes, marking, attendance records etc.

    Why we need it

    We need to know your personal data in order to fulfil our contract of services with you in line with our standard booking terms and conditions. This will be processed under GDPR legal basis 6(1)b ‘necessary for the performance of a contract’.

    What we do with it

    All the personal data provided by you is used to create an online account and registration onto a short course. Your data will be processed by UAL Short Courses Ltd as the ‘Data Controller’ and passed to Flywire Payments Ltd who will act as ‘Data Controller’ to process payment for the course.

    Now you need to upload a copy of your purchase order or a sponsor letter.

  • Please ensure you state the PO number before returning this form. This must be entered now to be included in the booking; it cannot be entered at a later date.

  • If you do not have a purchase order, please attach a sponsor letter from your company on headed paper.

    Accepted file types: pdf, doc, docx, Max. file size: 10 MB.
  • Invoice and copy of booking form will be emailed to this address (must be different from delegate email address).

  • You can use this form for up to 3 candidates, taking up to 2 courses each.

  • Booking Confimation

    Once we receive your booking form we will book the candidate onto the course and invoice your company finance department for payment. While we endeavour to run all short courses, please note that some courses may not run at the scheduled time. We strongly recommend that travel is not booked and course materials are not bought until at least 14 days before the start of your course and we have confirmed that the course will be running.
  • You must agree to pay within 30 days of the invoice date. Bookings made less than 14 days prior to the start of the course will require immediate payment.

    Please include the candidate's name and 'UAL Short Courses' in the transfer reference. Failure to do so may cause a delay in confirming the payment and place on the course.
